Those are mods made by Hexagonal-bipyramid.
The needs are locked because I've installed the blind needs challenge mod.
The portrait is different because I've installed this UI poses default. There's quite a lot of them, and if you would like something else - here's a wonderful tutorial about creating your own UI poses default.
