Default Worlds/Neighborhoods
I'm pretty confused with how this world-thingy is working. Is it like in sims 3 where you generated a new world and neighborhood with the same NPCs or is it like in sims 2 where you played in the same neighborhoods and if you messed up an NPC that was it?

(I hope you understand what I'm trying to ask)

It can be both. So you can have multiple saves with the same people or you can do rotational play.
