For arms, it's likely easier if it is part of the outfit and not an accessory, but you can make both options. Defaults would not work, since all sims would end up in clothes missing arms.
Also keep in mind that armless meshes will look strange when sims are carrying something, and that rings will float in mid-air if the sim is married/engaged.
For nude clothes, you can make showerproof skins (there are several examples over at Insim on how those work).
But start out just making the clothes.
The easiest way to close up the limb is to do so where there is skin texture. Don't make it hard for yourself (as a beginner) by trying to remove it on a mesh that's got clothes in the cutoff point, because those would look different. Remove the hand, make a rounded shape with what you have left, and make sure to fix the bone assignments around the stump after.
There are three tutorials called "Unimesh" here at MTS. Those should get you through the beginner stages of editing clothing/CAS meshes (these use Milkshape - if you want to use a different program instead, I think there are tutorials for Blender somewhere, too).
