Default Inactive sim died on community lot, no grave
One of the sims from an inactive household in town died after getting struck by lightning on a community lot (while talking to an active sim). The gravestone appeared on the ground on the community lot while the Grim Reaper was around, then disappeared. It was placed close to the very edge of the lot, but I'm not sure if it fell within the lot or not, maybe that made the difference.

It is not in the Mausoleum. A couple of days have gone by in-game. The death was registered, the family was grieving for them.

I've tried looking for the grave with Nraas Master Controller with the following:

Town Hall -> Town -> Object Info -> Not Inventory and Town Hall -> Town -> Object Info -> Inventory: There are various graves in town (Objects.UrnstoneHuman) but not this sim's.
Town Hall -> Sim -> Basic -> Find Grave: This sim is not shown in the list of dead sims whose graves I can look for.

So I looked for the sim themselves:
Town Hall -> Sim -> Status -> Personal -> Type of Sim -> Dead - they do appear there, so they're not completely lost. From their short status I can't find differences between this sim and other dead sims who used to be in inactive households. Their thumbnail is not a ghost, but that seems to be the case for some sims. Death by electrocution is listed.

I've also tried:
- saving, quitting the game, and restarting; then looking in the mausoleum and through the Master Controller options once again - same result.
- resetsim <name>; then looking in the mausoleum and through the Master Controller options once again - same result.
- removing all mods and CC, then looking at the mausoleum and on the ground on the community lot where the death happened. Not there. I don't believe I had anything to specifically interfere with death actions in the first place.

Any help would be much appreciated!

That usually happens when a sim dies on a community lot, especially in that scenario - the sim shows as dead, but there is no grave. It usually means the grave object is gone, not the sim.

First try using MasterController:
1) Town Hall > NRaas > MasterController > Sim > Advanced > Force Kill
2) Select the dead sim and choose the same death type.

This does not actually kill them again, because they’re already dead. It just forces the game to generate a new gravestone object. After that, the grave should appear normally.

If that doesn’t work you can try:
1) NRaas > MasterController > Add Sim. Add the dead sim to an active household.
2) NRaas > MC > Advanced > Reset Sim. Sometimes that makes the game recreate the urn.

Note: But before trying this you should make a copy of this save file or a backup, just in case if anything goes wrong.

Straight Force Kill from Town Hall doesn't list the sim as an option - the list is the same as the one for "Find Grave". But adding the sim to the active household gets me to the ghost, with the grave in their inventory. Force Kill from there will place the grave in the Mausoleum - I couldn't think of another way to get it out of inventory, there may be a more elegant solution

As for Reset Sim - I haven't managed to try it yet - the game bugged out straight after this procedure, so good call on having a back-up save... I expect it will work after a few restarts or something, will update this post when I succeed

It didn't even occur to me that disappearing graves might be just what happens in Sims 3, and that everybody else I've seen die/dead so far might have always died on their own lot or while visiting a friend
