Something happened at some point in the process that inversed the normals of the mesh.
If there's a solution in Milkshape I wouldn't know it, as it's quite an outdated tool to use for anything more than importing/exporting certain formats.
You can fix it in Blender by exporting the mesh from TSRW as .obj, import into Blender, Edit Mode, select all, ALT N to open the Normals menu, Average > Face Area. Export as .obj with the settings 'Selection Only' and 'Objects as OBJ Groups' ticked.
Import into TSRW again. Afterwards, you'll want to click the bone icon next to the import/export icons and copy them from any object, as .obj format loses the bone assignment.
