This user has the following games installed:

Sims 2, University, Nightlife, Open for Business, Seasons, Bon Voyage, Free Time, Apartment Life
Default Plastic surgery being permanent?
I have the mod where I could spawn the 'Social Adjuster', so I don't use the original plastic surgery machine. The object for the adjuster are those base game party balloons.

So, I had a sim with a sharp hollow face that got pregnant and wanted to make her face a bit swollen for storytelling purposes and revert it back to her original face when she "recovers". However, when one of the twins aged into toddlers I noticed her inheriting the chubby cheeks that were given through plastic surgery. This happened once more when my fat sim had a baby and the toddler inherited the sim's "fat face" instead of the sims' original face.

I'm not entirely sure which mode it is because I had it for years, I think it has to do with Insimenator, Inteen or something. Is plastic surgery through that mod actually permanent? Any experience? Honestly, I would love if it was.

Did you have a look at the other sim's (dad?) face? The chubby cheeks could be from them.
If you want the changes made permanent, you'll have to do so with the simsurgery method in SimPE. The plastic surgery is only cosmetic, and doesn't change the "genetic face".

No, both parents had a chiseled face. I'll actually do a test on different families and post results. Maybe this mod works differently than the maxis plastic surgery.

Yes, I know about the SimPE surgery but it's for copying an entire face, not tweaking an existing face.

Could be a broken face template, or parts of one.

You tweak the face ingame (or by extracting the sim via SimPE/Bodyshop), then use the option in SimPE to copy over the face from the edited sim to the old sim, resulting in a genetic face. You don't actually do the tweaking in SimPE.

When you use the surgery dialog inside the game (from any object), it creates a new facial structure LXNR with instance number 2 alongside the base one with number 1. The person then has two faces. Supposedly face 1 is what is used for creating children. 1 can be replaced with 2 to make it permanent, or either facial structure can be copied to another person, then the face updated at a mirror to combine it with hair/glasses/clothes. Structure 2 may likely be deletable to undo the surgery, but I have never tried that.
