Anyone had this glitch before? My servo has two faces layered on top of each other-- one generic sim face, on top of her regular servo face. Leaving her looking like a half-sim/half-robot monstrosity (screenshot attached). I'm relatively certain this was caused by me changing her outfit (and/or the mirror?), but I've changed my servo outfits to other servo-matching clothing hundreds of times before and this usually doesn't happen even if the mod I'm using opens the mirror for my servo automatically... and I truly can't figure out how to change it back. If I open her up in the mirror and close it, she stays the same, and if I try to change her appearance in the mirror she does end up with only one face... but it's the sim face.

If I have to, I'm willing to just drop a satellite on her and let her be a creepy-looking glitched ghost... or maybe just let her take the sim face and give her a sim-body mesh outfit to match and she can be a robot in disguise as a sim despite that not being something I usually do... but if there's a fix, I'd rather just NOT do either of those things. Anyone have any ideas?

Have you tried changing the servo's outfit back to everyday (or switch forth and back)? Apparently this is one of the things that can fix the issue.

I have tried switching between everyday and formal, but both her everyday and formal outfits are custom servo-mesh outfits. Pictured is what I have set as her everyday wear.

Found the solution! I transformed her into a witch because I remembered that updates servo faces to have the witch hat, and then I changed her back to a servo. It set her back to the default outfits with the regular servo face and then I just set her servo-body custom outfit again and she's back to normal!

Just a tip for future clothing changing - both the SimBlender and Insimenator (Wardrobe adjuster) have the option to only change the outfit, and there are other mods that allow this as well. Might be better to use one of those mods for servos, than the one that auto-shows Change Appearance.
