Default Sims can't stop hating/attacking each other
Sims keep randomly insulting and annoying each other to the point that almost every Sim hates each other. At first, I was fine with it because I figure it'll be interesting to have some enemies here and there. But then it seemed like every sim hates each other and at one point, two sims that were flirting for one second starts to insult each other the next and then go back to flirting like that last interaction didn't happen (kinda funny but still).

I thought ACR have something to do with this, but after some thinking and checking the ACR settings, I highly doubt it. It must be a mod conflict. Perhaps the "insults unlocked" mod or "fight club" (even though I need those mods for story telling purposes).

Both mods sound like they could be potential culprits.
It doesn't have to be a mod conflict, though - could just be intended mod functions with unwanted consequences.

Whichever mods they are, make sure to read through the info (look them up and read through them), especially anything about mod fuctions, potential conflicts and if you also need supportive mods (such as Smarter EP check, or similar).

If sims already have a low relationship (+/- 0) and then start throwing insults around, their relationship more easily go in the red, which is often harder to get up to green, especially if they do all the interactions autonomously. For grouchy sims possibly even more so. Having very different interests and negative chemistry also doesn't help. Autonomous insults/fights do happen even without mods, but if the ones you've got increase the chance, you've got yourself a relationship crisis waiting to happen.

I've been having the same trouble and was about to ask for advice. It seems any sim with under 4 points of niceness will annoy and insult other sims the moment they meet; then as soon as they have a negative relationship with someone, they follow them around community lots poking them until they start a brawl, preferring bothering people to fun objects or food on the lot; very quickly they gain a negative reputation, and then any sim regardless of niceness points will yell at them and cause negative relationship, and it snowballs from there. Like you I want to have a bit of chaos and don't need everyone to get along, but it's become difficult to micromanage sims out of bad reputations they've given themselves to prevent endless brawling.

I don't have Fight Club but do have HB's Mean Interactions to Any Age - original version, before the update separating out mean actions into several little mods. I had wondered whether the mod was the culprit, but it's not the "any age" part that's a problem - adults are perfectly able to start wars with each other without involving any teens. Maybe it has some other effect too?

I've frequently seen this noted as an effect of the mean interactions to any age mod. Fight club doesn't change when any interactions are available, it only changes how fight outcome is calculated, I believe.

They always like Irritate actions and poke repeatedly. The poke has diminishing returns, so the first person keeps poking without escalating to a fight unless the other person can do an action in return after a pause. Usually the second person cannot because they have a period of crying. I haven't noticed an effect of niceness because almost nobody has a high nice. With a normal set of people, Irritating is like a drug.

I have actions open up between teens an adults, but that only increases the likelyhood of them happening on a lot with mixed population. If I had a lot with all adults, this would stil happen the same. I feel like there should be a difference in behavior built in, because adults in reality are more restrained.

At one point I wanted to mod this but lost interest. Maybe make it so that he checks in his diminishing returns array if he has already done a poke.
